Q: I work on a ranch in California and have noticed that one of the horses is slightly lame. He is a three-year-old dapple grey, thoroughbred gelding. His left hind fetlock has swollen to double its size. He had an old scab at the base of the fetlock, and all the hair is matted and ruffled. It doesn’t appear to be sore or stiff, despite the swelling. Would you suspect this swelling is caused by the cut or a more serious lameness?

There are two main possibilities here. One is that your horse has pastern dermatitis which has resulted in swelling in the limb. This is the result of a bacterial infection in the base of the hair follicles and is common in horses turned-out at grass.

The second possibility is that your horse has developmental orthopedic disease in the joint. Osteochondrosis can produce excessive synovial effusion and cause the joints to swell.

My advice would be to have your horse examined by a veterinary surgeon who will be able to determine whether the swelling is caused by infection external to the joint, or by excessive fluid within the joint itself.

At that stage, you can decide whether you want to have radiography carried out on the joint. This will give a firm diagnosis and prognosis of the lameness.
